IF YOU WISH TO USE ALLEGHENY HEALTH NETWORK MYCHART, YOU MUST READ AND AGREE TO THE FOLLOWING PATIENT PORTAL TERMS AND CONDITIONS OF USE:


Welcome to Allegheny Health Network MyChart (the “Patient Portal” or the “Service”). This Patient Portal is an Internet application that allows secure web-based communication between a patient and Allegheny Health Network or other health care provider participating in the Patient Portal. The Patient Portal allows patients to communicate with their health care providers regarding non-urgent issues, schedule appointments, pay bills, and also view parts of their Allegheny Health Network electronic medical record. Enrolling in the Patient Portal is optional; however, certain features like eVisits and video visits(described more fully below)require the use of the Patient Portal.

BY CLICKING “ACCEPT” AT THE END OF THIS DOCUMENT, YOU ARE AGREEING TO BE BOUND BY THE PATIENT PORTAL TERMS AND CONDITIONS OF USE. IF YOU CHOOSE NOT TO PARTICIPATE IN THE PATIENT PORTAL AND/OR DO NOT AGREE TO THE PATIENT PORTAL TERMS AND CONDITIONS OF USE, YOU MAY CLICK "DECLINE". IF YOU DO CLICK “DECLINE,” YOU WILL NOT BE GRANTED ACCESS TO THE PATIENT PORTAL OR ANY OF ITS FEATURES. Portions of your medical record are stored by Allegheny Health Network on the Patient Portal. By using the Patient Portal, you are confirming the accuracy of all the personal data you have provided for enrollment in the Patient Portal. You are also confirming your request for, and authorization of, the release of those portions of your medical record that you wish to view. Only certain information from your medical record will be available for your review via the Patient Portal. Your full medical record maintained by Allegheny Health Network can be requested through our Health Information Management department, which you may contact by calling 412-330-5399. The release of your medical record is subject to Allegheny Health Network’s policies and procedures, appropriate authorizations, and applicable law. If you wish to revoke your authorization for release of health information via the Patient Portal, you must cancel your enrollment using the method described herein.

  • Allegheny Health Network is an integrated healthcare system consisting of primary and specialty care practices, hospitals, urgent care centers, outpatient surgery sites, diagnostic testing centers, and home health and medical equipment providers. As participants in an Organized Health Care Arrangement (OHCA), our providers may share your information for healthcare operations of joint activities.
  • Allegheny Health Network MyChart is owned and maintained by Allegheny Health Network.
  • Allegheny Health Network may be referred to as "we", "us", or "our" throughout this document.
  • The term "you" or "your" refers to Allegheny Health Network-authorized viewers or users of the Patient Portal.

PART I: AVAILABLE SERVICES

1. Accessing your health information. The Patient Portal provides you with secure access to portions of your medical record created by Allegheny Health Network and other healthcare providers. Not all of your health information is available through the Patient Portal, including records maintained by non-participating providers. Because the information available to you does not represent the totality of your medical record, it should not be relied upon as a complete or up-to-date representation of your health status.

2. Health-related links. In order to provide you with additional resources, the Patient Portal may contain links to Allegheny Health Network and external websites or services. By providing links to an external site or service Allegheny Health Network makes no representations or warranties regarding such site or service. While we do select the provided links with care, we cannot guarantee the accuracy, security, or usefulness of the information within those sites. We do not accept responsibility for external sites, and you should not rely on the information found on external sites in lieu of seeking treatment or diagnosis for a medical condition. When you do choose to link to an external site, you will be clearly advised that you are exiting the Patient Portal.

3. Virtual Visits/Telehealth The Patient Portal allows you to request eVisits and video visits (collectively “Virtual Visits” or “Telehealth”) from your home. By initiating a Virtual Visit, you will interact with a participating provider through the Patient Portal to assess certain non-emergent conditions. You will communicate with the provider either by secure messaging or a real-time video chat. For more details regarding Virtual Visits with Allegheny Health Network, including associated costs, treatable symptoms, hours of service, etc., please navigate to the appropriate link after logging into the Patient Portal. Please be aware that you must be a resident of the Commonwealth of Pennsylvania to participate in Virtual Visits.

Laws protecting the privacy and security of health information apply to Telehealth services, and information exchanged during Virtual Visits will be transmitted over a secure network and stored in a secure data repository. Please be advised that there may be risks associated with Telehealth, including the quality of transmitted information, the ability to fully evaluate and diagnose certain conditions, delays in consultation due to equipment limitations, and in rare cases, a security protocol failure.

Virtual Visits are not suitable for treating serious conditions or medical emergencies. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY OR SERIOUS CONDITION, CALL YOUR DOCTOR'S OFFICE, VISIT THE NEAREST EMERGENCY DEPARTMENT OR URGENT CARE CENTER, OR DIAL 911 IMMEDIATELY.

4. Online bill payment. The Patient Portal allows you to pay your healthcare-related expenses online. Allegheny Health Network adheres to Payment Card Industry (PCI) information-processing and security standards, and securely maintains your financial information within the Patient Portal.

5. Family Access The Patient Portal offers the option of designating a proxy to be granted account access (“Family Access”). A parent or legal guardian of a minor under the age of 13 can establish Family Access for himself/herself without permission from the minor; minors between the ages of 13 and 17 can establish Family Access for a parent or legal guardian; and individuals age 18 and older can establish Family Access for any other adult involved in his/her care. For more information regarding Family Access, including revocation, and to locate the appropriate form(s), please navigate to the appropriate link after logging into the Patient Portal.



PART II: UNINTENDED USE

1. Should not replace your provider. The Patient Portal provides convenient access to your health information and secure communication tools, but it is not designed to replace your health care provider. It is intended to enhance and supplement your healthcare experience and your role in managing your health, but it should not eliminate ongoing health maintenance or wellness activities, such as regular office visits/check-ups with your provider. You should not stop or change any course of treatment ordered by your provider without first consulting him/her.

2. Should not be used for seeking emergency care. EMERGENCY CARE IS NOT AVAILABLE THROUGH THE PATIENT PORTAL. IF YOU THINK YOU HAVE A MEDICAL EMERGENCY OR SERIOUS CONDITION, CALL YOUR DOCTOR'S OFFICE, VISIT THE NEAREST EMERGENCY DEPARTMENT OR URGENT CARE CENTER, OR DIAL 911 IMMEDIATELY.

3. No access to health information from non-participating providers. Some of your health-care providers may not use the Patient Portal, and thus they may not be aware of some or all of the health information that is in your account. It is your responsibility to inform ALL of your health-care providers about consults with, and courses of treatments recommended by, other providers, as well as any test results and other matters related to your health status.

4. Access for minors. The Patient Portal is generally not intended for use by minors. In accordance with Pennsylvania law, minors between the ages of 13 and 17 can consent to certain healthcare services independently, and also control access to information related to those services. In those situations, individuals at least 13 years of age may request a Patient Portal account, and also revoke prior Family Access rights of a parent or legal guardian. However, unless otherwise permitted, Allegheny Health Network does not knowingly collect personal information from a minor under the age of 18. If Allegheny Health Network determines that someone under the age of 18 has impermissibly created an account, it will be disabled.



PART III: YOUR RESPONSIBILITIES

A. Monitoring account for messages and notifications. Allegheny Health Network and other providers may send you secure messages or notifications that contain important health care information via the Patient Portal. For example, your provider may send you a message regarding recent test results, or you may receive a notification that you are due for a flu shot or other preventive services. It is your responsibility to monitor your account to review these messages and notifications.

B. Safeguarding your email account. You will be notified via email when new information is available on the Patient Portal, such as a billing statement. Therefore, any person with access to your email account would also be able to review these notifications. This could include your spouse, employer, or anyone else with access to or oversight of your email account. Although no specific health information will be included in the email, the notification alone may be information that you prefer to be kept confidential. Please consider this when providing an email account for enrollment in the Patient Portal.

C. The Patient Portal is for your health only By clicking “ACCEPT” below, you agree that all communication through the Patient Portal will be about you and your own health. However, if you have been designated as a proxy with Family Access on someone else’s account, you are permitted to communicate about that individual and his/her health. You also acknowledge that the content of any message sent via the Patient Portal may be stored in your medical record, or the medical record of the individual on whose behalf you are communicating. Providers will not comply with any request for advice or information regarding another person, and such a request is a violation of this Agreement. Allegheny Health Network does not assume responsibility for health information used by persons other than the Patient Portal enrollee.

D. Cancelling Enrollment. If you decide to discontinue your use of the Patient Portal, you must call your provider’s office to cancel your enrollment. Should you cancel your enrollment, you will no longer be able to use or access the Patient Portal, or any of its features, such as Virtual Visits or online bill pay, and your account on the Patient Portal will be closed. Any information stored in the Patient Portal at the time your account is closed will be archived in our database per applicable recordkeeping requirements.

E. Requesting updates to health information. You have the right to request updates to your health information via the Patient Portal. For example, if your account is displaying a medication that you are no longer taking, you can securely message your provider to ask that your account be updated accordingly.

F. Acceptable use. Upon creating a unique username and password, you will be granted access to the Patient Portal. Your use of the Patient Portal must be in accordance with these Terms and Conditions of Use, and in compliance with all applicable laws, rules, and regulations. Failure to abide by applicable standards of acceptable use may result in Allegheny Health Network terminating your Patient Portal access privileges.

G. Responsibility for activities related to username and password. You are solely responsible for any activities related to your username and password. You must immediately report to Allegheny Health Network:

  • Any suspected or actual unauthorized access to your account;
  • Any loss or disclosure of your username or password; or
  • Any suspected or actual disclosure of health information.



PART IV: PRIVACY AND SECURITY STANDARDS

This section describes the privacy and security controls in place to protect your information, and the steps you should follow to ensure that your information remains confidential and secure. If you are a patient of an Allegheny Health Network facility, you have certain rights associated with your health information. These rights are established under state and federal law. For more information regarding patient rights and how we may use and disclose your health information in accordance with applicable law, please refer to the Allegheny Health Network Notice of Privacy Practices, which can be found on Allegheny Health Network's public website.

A. Unique username and password. Your username and password secure the confidentiality of your account and health information on the Patient Portal. You must create a unique username and password in order to access the Patient Portal. DO NOT share your username or password with anyone. Sharing your username or password may compromise the confidentiality and security of your information on the Patient Portal.

B. Location appropriateness. You are solely responsible for choosing an appropriate location and connection method from which to log into the Patient Portal. Please be advised that you may compromise your information by accessing the Patient Portal in a public space (e.g., a library) or over an unsecure network (e.g., open wireless). You are solely responsible for any intentional or unintentional disclosure of your health information which may result from using the Patient Portal in a public space or over an unsecure network.

C. Log-out. You should always log out of the Patient Portal after you have finished using the Service, and especially when you are accessing the Patient Portal outside of your own home or through a shared device that does not belong to you.

D. Secure connection You can determine if your access to the Patient Portal is secure by looking at the location (URL) field in your web browser. If the URL begins with “https://” (rather than “http://”), the session originates from a secure server. This means your data is encrypted and cannot be read or deciphered by unauthorized individuals.

We do not sell or rent any personal information we receive through your use of the Patient Portal. The Patient Portal has security measures in place to help protect against the loss, misuse, or alteration of information under our control. These measures include encryption of data using Secure Socket Layer (SSL) technology and the use of a secured messaging service. Despite these measures, the confidentiality of any communication transmitted via the Patient Portal by Internet or email cannot be guaranteed. There is always the possibility that an unauthorized person may intercept, read, and/or modify any email sent to or received by Allegheny Health Network.

E. Evaluation. Users may periodically be asked to complete surveys regarding their experiences with the Patient Portal. Surveys are completely voluntary and are only for the purpose of providing feedback and improving the quality of the Service. Our surveys may ask participants for demographic information such as age, gender, and education, but will not request that users provide specific information regarding any medical condition. In addition, users who provide feedback may be individually contacted for follow-up due to particular concerns raised during the course of such evaluation.



PART V: MISCELLANEOUS

A. Right to terminate access privileges. Allegheny Health Network reserves the right to terminate your access to the Patient Portal at any time, for any reason, and at its sole discretion. You will be notified if we terminate your access to the Patient Portal.

B. Patient Portal is a private network. The Patient Portal is a private Internet application provided and maintained by, and proprietary, to Allegheny Health Network on behalf of itself and its subsidiaries and affiliates. Unless we have specifically authorized you to access the Patient Portal, it is a violation of state and federal law to attempt to access the private computer network of another individual or entity. Use of another individual’s username and password to access the Patient Portal is not permitted and does not amount to authorization to use the Patient Portal.

C. Updating the Terms and Conditions of Use. Allegheny Health Network reserves the right to amend, modify, or promulgate additional Terms and Conditions of Use pertaining to the Patient Portal at any time. Revisions will be effective immediately upon publication, and will apply to your information which we have already received, as well as to any information created or received moving forward. We encourage you to periodically review the Terms and Conditions of Use. The date of the most recent revision will be clearly posted at the end of this document. Your continuing use of the Patient Portal constitutes your acceptance of and agreement to comply with all such rules.

D. Patient Portal improvements. We reserve the right to make changes, updates, and/or improvements to the Patient Portal platform as well as to any products and/or services provided on the Patient Portal at any time.

E. Monitoring, investigations, security review and audit. Allegheny Health Network reserves the right to monitor all activity occurring on the Patient Portal as well as any information stored in or transmitted through the platform. By clicking “ACCEPT” below, you agree to cooperate fully with any investigations, security reviews, or audits conducted by Allegheny Health Network related to the use of the Patient Portal.

F. Minimum system requirements. You can access the Patient Portal via a personal computer or lap top computer, provided the computer is running Windows 2000 or later, Internet Explorer Explorer 8.0 or higher (other browsers are not supported at this time), and Adobe Reader/Professional 8.0 or higher. At least 512MB of RAM is recommended to support full functionality. You are solely responsible for keeping your computer secure, including updating it and its applications with required upgrades and security patches as available.

G. Patient Portal unavailability. There may be times when the Patient Portal is unavailable due to technical complications or during maintenance activities. We will make reasonable efforts to limit system maintenance to off-peak hours.

H. WARRANTY DISCLAIMER. THE PATIENT PORTAL AND ITS CONTENT AND SITE-RELATED SERVICES ARE PROVIDED "AS IS," WITH ALL FAULTS, WITH NO REPRESENTATIONS OR WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. YOU ASSUME TOTAL RESPONSIBILITY AND RISK FOR YOUR USE OF AND RELIANCE UPON THE INFORMATION CONTAINED WITHIN THIS SITE, SITE-RELATED SERVICES, AND HYPERLINKED PATIENT PORTALS. NO ORAL OR WRITTEN INFORMATION OR ADVICE GIVEN BY ALLEGHENY HEALTH NETWORK OR ITS AUTHORIZED REPRESENTATIVES SHALL CREATE A WARRANTY NOR IN ANY WAY SHALL IT INCREASE THE SCOPE OF THIS WARRANTY.

I. LIMITATION OF LIABILITY. ALLEGHENY HEALTH NETWORK, ITS PARENT, SUBSIDIARIES, AFFILIATES, AND THEIR RESPECTIVE OFFICERS, TRUSTEES, DIRECTORS, EMPLOYEES, AGENTS AND CONTRACTORS, AND ANY OTHER THIRD PARTIES MENTIONED ON THIS SITE ARE NEITHER RESPONSIBLE NOR LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, PUNITIVE, OR OTHER DAMAGES (INCLUDING, WITHOUT LIMITATION, THOSE RESULTING FROM PERSONAL INJURY, LOST PROFITS, LOST DATA, OR BUSINESS INTERRUPTION) ARISING OUT OF OR RELATING IN ANY WAY TO THE SITE, SITE-RELATED SERVICES AND PRODUCTS, CONTENT OR INFORMATION CONTAINED WITHIN THE "SITE," AND/OR ANY HYPERLINKED PATIENT PORTAL, WHETHER BASED ON WARRANTY, CONTRACT, TORT, OR ANY OTHER LEGAL THEORY AND WHETHER OR NOT ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. YOUR SOLE REMEDY FOR DISSATISFACTION WITH THE SITE, SITE-RELATED SERVICES, AND/OR HYPERLINKED PATIENT PORTALS IS TO STOP USING THE SITE AND/OR THOSE SERVICES. IF, AND TO THE EXTENT APPLICABLE, LAW DOES NOT ALLOW THE EXCLUSION OR LIMITATION OF INCIDENTAL OR CONSEQUENTIAL DAMAGES, THEN THE ABOVE LIMITATION OR EXCLUSION WILL BE LIMITED AS REQUIRED BY APPLICABLE LAW.

J. Copyrights. Except as otherwise indicated, all content on this site is the property of Allegheny Health Network and/or its licensors and is protected by law and may not be used by you except in connection with your usage of the Patient Portal in accordance with these Terms and Conditions of Use.

K. Trademarks and service marks. The names, words, symbols, and graphics representing Allegheny Health Network and the Patient Portal are the trademarks or registered trademarks of Allegheny Health Network and are protected by trademark laws of the United States of America and of other countries. Other proprietary names, words, symbols and graphics may be designated as such from time to time on this site through use of the TM, SM, or ® symbols. Users of the Patient Portal are not authorized to make any use of the Allegheny Health Network marks, including, but not limited to, as meta-tags or in any other fashion which may create a false or misleading impression of affiliation or sponsorship with or by Allegheny Health Network.

L. Accuracy and integrity of information. Although we will attempt to maintain the accuracy and integrity of the Patient Portal, we cannot guarantee that the information will always be correct and complete. It is possible that the Patient Portal may include typographical errors, inaccuracies, or other mistakes. In the event that you do notice an inaccuracy or mistake, please inform us so that we can make the proper correction(s).

M. Infected or corrupted materials. To the extent files are available for download through the Patient Portal, Allegheny Health Network or any participating provider cannot and does not guarantee or warrant that such files will be free of infection or viruses, worms, Trojan horses, or other code having contaminating or destructive properties.

N. No waiver. No delay or failure by Allegheny Health Network to require performance of any provision of these Terms and Conditions of Use shall constitute a waiver of the performance of such provision. Any waiver granted by Allegheny Health Network must be in writing, and shall apply solely to the specific instance expressly stated. A waiver of any specific term or condition shall not be construed as a waiver of any other provision of these Terms and Conditions of Use, nor shall any waiver constitute a continuing waiver.

O. Choice of law. By using the Patient Portal, you agree that the laws of the Commonwealth of Pennsylvania, without regard to principles of conflict of laws, will govern these Terms and Conditions of Use and any dispute of any sort that might arise between you and Allegheny Health Network or any of its subsidiaries or affiliates. You agree not to commence or prosecute any action in connection therewith other than in the state and federal courts of Pennsylvania, and you hereby consent to, and waive all defenses of lack of, personal jurisdiction and forum non conveniens with respect to venue and jurisdiction in the state and federal courts of Pennsylvania.
